Please provide the following if your ID card is lost, stolen or confiscated.

  • Police Report
  • Report From an Installation Security Office
  • Statement/Document(s) as Provided by the Service/Agency (e.g., Counseling Document)
  • Letter or memo from the member's security manager, Commander, First Sergeant, or the sponsoring TASS Trusted Agent. The letter or memo must be prepared on the activity's letterhead, dated and signed with a wet signature or digitally signed using a CAC.

Please print the LOA (orders) and bring with you prior to arrival at MPF.

  • Dependents must be accompanied by their sponsor. If not, a completed DD Form 1172-2 must be either wet signed by the sponsor and notarized or digitally signed (using a CAC) (within the past 90 days) or the dependent must have a current Power of Attorney.
  • Two valid forms of government ID for any one over the age of 18.
  • Renewals can be made at the kiosk or by making an appointment.

  • Provide a school enrollment letter stating they are full-time and have their graduation date. If the sponsor is not accompanying the dependent, the DD Form 1172-2 must state in Item 21 (Remarks) that “the sponsor is 50% and over responsible for the dependent”

  • 2 Valid forms of Government ID

  • Unmarried biological, step-children and adopted children are eligible for TRICARE until age 21 (or 23 if: Your child is enrolled in a full course of study at an approved institution of higher learning and you (the sponsor) is still providing more than half of their financial support).

  • Eligibility may extend beyond these age limits if he or she is severely disabled.

  • At age 21 or 23, he or she may qualify to purchase TRICARE Young Adult.

  1. Must have been married for at least 20 years
  2. Spouse must have served in the military for at least 20 years
  3. 20 years of the marriage must overlap 20 years of the spouse’s military service

If a military spouse meets all of these qualifications, they get access to the same benefits as the military member for the rest of their life—as long as they don’t remarry. This means that they receive Tricare, commissary and exchange privileges, and a portion of the spouse’s retirement pay. They may lose these benefits if they marry someone who is not a military member.

REQUIRED DOCUMENTS:

  • Statement from former spouse that he/she does not have employer sponsored health insurance and has not remarried (this will also be put in the Remarks section of the DD Form 1172-2 and the applicant MUST INITIAL)
  • Once documents are compiled.  Customer Support will send all the documents off to the applicable project office.  They have up to 60 days to approve/disapprove.

    • Please allow up to 30 business days for your USID card to be created.
    • You will receive an e-mail from: 6fss.fsmps.deersidcards@us.af.mil.
    • Please ensure you check your SPAM/Junk folder.
    • Either the sponsor or the dependent can pick up the card. Please see below on what the dependent needs to bring if picking up without the sponsor.

  1. When your new ID card is created, your old ID card will be terminated.
    • Please visit the MacDill AFB Visitors Control Center to receive a base pass. The Visitors Control Center is located next to the MacDill AFB Main Gate.

  1. Visit the 6 FSS Military Personnel Flight to pick up your new ID card!
    • Please ensure the following:
      • Bring your expired ID card to turn in.
  • Bring your signed/notarized DD Form 1172-2 OR ensure that your sponsor is accompanying you to sign the DD Form 1172-2 OR bring your current power of attorney to sign the form.

    • Hours of Operation for picking up an ID Card: Monday – Friday, 0800-1500 (unless holiday or other wing closure day)

The sponsor or the dependent can pick up ID cards after using the kiosk. If the sponsor DID NOT sign a Form 1172-2 then the sponsor will need to be present to sign the form when picking up the card. If the sponsor is not available, the dependent will need a current power of attorney to sign for them when picking up the card. If the DD Form 1172-2 was signed (digital signatures for CAC holders or notarized for wet signatures) and scanned at the kiosk or uploaded to DEERs beforehand, the dependent can pick up the ID card without the sponsor being present. The sponsor is always welcome to pick up on behalf of their dependent(s). 

Yes, bring the death certificate (in case it is not updated in DEERS). These members can utilize walk-in hours for this service. They will then become their own sponsor. 

Yes, the soonest you can apply for an ID card is 30 days on the kiosk and/or when the member receives their Medicare A or B cards which can be uploaded using the kiosk.

BOTH KIOSKS ARE LOCATED INSIDE THE BX ACROSS FROM THE FOOD COURT

Only sponsors with CACs can submit DD Form 1172-2 to the DEERS online self-service portal (This link requires CAC enabled computer) ahead of time. All other sponsors should download the 1172-2 and sign in front of a notary before scanning into the kiosk. The UPS store inside the BX offers notary services for a nominal fee.

Bring with you 2 forms of ID, follow the questions on the screen, scan your documents, take a photo and hit submit.

Your request will be sent to the ID Card office on MacDill AFB for renewal. You will be able to pickup your ID card within 30 business days.
